Default I didn't know 100% that my boyfriend was the biological father

Georgia

If I didn't know 100% that my boyfriend was the biological father of my son when he signed the paternal acknowledgement form and my son's last name was changed, could we get in trouble?

In trouble, depends on how you define it.

In GA, if the acknowledgement is signed but later there's a question of paternity

"Either the mother or biological father has 60 days from the date of his/her signature to request to rescind this PA. After the 60 day rescission period has ended, this signed document may constitute a legal determination of paternity and can only be challenged in a court of law on the basis of fraud, duress, or material mistake of fact, with the burden of proof on the person challenging the acknowledgement."

Depending how long it's been and dad's relationship with the child, he could be found the legal father, regardless of DNA. Happens all the time.
