Default Deregister (exclude) an employee from Public Health Insurance in Germany

Please inform me how many reasons may a company have in order to deregister (exclude) an employee from Public Health Insurance in Germany. Once excluded (wrongly) is it possible (and how) to register the employee again to Public Health Insurance in Germany. What kind of information should the health authority give to the employee in order to inform him/her about the exclusion from Public Health Insurance (and/or the registration to the Private one)?

If your gross income is less than €60.750 per year, or €5.063 per month, it is compulsory to be insured under a public, statutory health insurance scheme, and you must choose an insurance provider. Earning above the annual income limit (compulsory insurance limit) could be a reason for de-registration. If you are not, getting a confirmation letter from the benefits agency to say you are not claiming surely would help shatter their argument you earn over the limit.
